All transcript requests must be in writing. We require your signature. You may mail your request to: 800 College Ave, Jacksonville, TX 75766, or fax your request to: 903-589-1010. Each request requires the following information:

Student Name - printed (and all names you have gone by)

When you attended LMC

Name and Address of where you want the transcript mailed

Daytime phone number in case we need to contact you
Each official transcript is $5. We cannot fax an Official transcript.
Payments can be made by check or money order, or you may put the payment on a debit or credit card. Debit/Credit Card payments require the following information:
